Attendee must attend entire webinar to receive any CE certificates. Attendees requesting a CE Certificate for Social Work must successfully submit a post test prior to receiving certificates.

Presentation Overview:
It surprises many Americans to learn that 25 percent of all deaths in the U.S. are Veterans.  That’s 1,800 people a day; more than 680,000 Veterans every year.  These heroic Americans deserve recognition for their military service – particularly at the end of life.  This presentation will focus on the specific issues and unique needs that Veterans may experience as they age and at the end of life.  Specific psychosocial considerations and interventions presented will help participants enhance their clinical knowledge and increase competency for working with and caring for Veterans and their families.  Presentation will also touch on how VA facilities, community hospice and healthcare programs and other Veteran service and community organizations can work together to ensure that excellent care is available for aging Veterans and their families.

The goal of this webinar is to improve the care the Veterans receive as they age and at the end of life.

Objectives:

• Understand systemic need for improved care for aging Veterans.
• Identify varying experiences of Veterans from different branches of service and war eras and increase understanding of unique considerations for working with aging Veterans.
• Identify effective interventions for working with aging Veterans.

Jennifer Sax is a dedicated human services and public health professional with more than 12 years of experience, Jennifer Sax has been working at Circle of Caring at Hospice of the Good Shepherd for almost 8 years.  A graduate of the University of Vermont in Burlington, Jennifer’s initial post-graduate career was as the Assistant Director of a 52 bed residential home for foster youth in San Diego, CA.  After 3 years on the west coast, she re-located to Boston and began the search for a career change.  Remembering her experience as a hospice volunteer while growing up in Portland, ME, Jennifer found a potential position with Hospice an appropriate fit.  She has been there ever since. A community educator at the Hospice for the past 6 years, Jennifer has written and spoken extensively to raise awareness about the importance of having “Intimate Conversations” about advanced healthcare directives, improving patient choice about end-of-life care options and the benefits of hospice care.  She was instrumental pursuing agency accreditation by the National Institute for Jewish Hospice and most recently headed up the veteran committee which led the Hospice to level 2 We Honor Veterans accreditation.

Chelsea Funk is currently Hospice of the Good Shepherd’s hospice-veteran partnership liaison to the Hospice and Palliative Care Federation of MA. She has been a social worker with Circle of Caring at Hospice of the Good Shepherd for nearly 3 years. A graduate of Boston College Graduate School of Social Work, Chelsea has experience in both outpatient and inpatient medical social work as well as adult day health. Chelsea specializes in clinical practice at the end of life and with older adults and families. In addition to her work with Veterans, she has a particular interest in trauma and also working with populations who have been traditionally disenfranchised by the medical system. Chelsea has studied abroad in both the Czech Republic and South Africa. In 2009 she was recognized with the Helen J. Crowley Memorial Award from Boston College for her excellence and leadership in working with older adults and families and last summer Chelsea was named Newton Tab’s “Person of the Week”.

Cancellation Policy:
The registration fee for webinars is non-refundable. Registrants who are not able to attend the webinar will receive audio & video files following the event. For clinical webinars, CE contact hours will not be available to those who are not able to attend the entire event.

In the unfortunate event that a webinar needs to be cancelled or rescheduled NAPGCM will notify the registrant by email and provide information on the new date or alternative arrangements. If the registrant is not able to attend the new date NAPGCM will refund the registration fee or place a credit towards a future event.
